Delivering a parcel yourself and obtaining signature?

If you're delivering a parcel to someone and want to get a signature to say its been received, are there any templates you can download for this type of thing if you're an individual or small business not a courier or postal company? And what sort of thing should be included on the form were you get them to sign?
